ANNUAL DINNER - Nuneaton Mens Hockey Club

Our Annual Dinner will be held at Ambleside Sports Club on Friday 16th April 2010.

Please complete the following form to order your tickets for the Dinner. READ THIS LETTER FIRST. Once payment is received, we will send the tickets to the address you provide - PAYMENT CAN BE MADE IN INSTALMENTS WITH YOUR MATCH FEES TO YOUR CAPTAIN - Tickets will be issued once fully paid for. Full payment is required by 31st March 2010 or your tickets will be voided and your seats lost. The collection of monies last year was far from ideal, and this year will be dealt with more professionally - so please bear the deadline date in mind when you book your tickets.

PLEASE ARRIVE BY 7.00pm IN ORDER TO BE SEATED AT 7.30pm.

There is a Disco afterwards, courtesy of CELEBRATIONS DISCO - running til late, with a late-ish Bar too.

Prices: £ 20.00 for Seniors (18 and over), £15 for under 18s

ANYONE ATTENDING THE DISCO, BUT NOT THE DINNER WILL BE ASKED TO PAY £ 3-00 AT THE DOOR BEFORE THEY ARE ALLOWED TO ENTER.

WE WILL HAVE BOUNCERS.

WE WILL ALSO BE HOLDING A RAFFLE AND A FEW OTHER "SURPRISES" ON THE NIGHT - PRIZE DONATIONS WOULD BE GRATEFULLY ACCEPTED. IF ANYONE HAS "CONTACTS" FOR FOOTBALL TICKETS, EVENTS etc, THESE WOULD BE MORE THAN WELCOME PLEASE.
